What is Spiriva?
Spiriva (tiotropium) is a potent bronchodilator expertly formulated to relax airway muscles, facilitating enhanced airflow to the lungs. This medication is primarily indicated for the prevention of bronchospasm in adults suffering from chronic obstructive pulmonary disease (COPD), including conditions like bronchitis and emphysema.
Additionally, Spiriva Respimat is effective in preventing asthma attacks for adults and children aged 6 years and above, although it is not approved for younger patients.
Essential Information
When using Spiriva, ensure that capsules are only utilized with the HandiHaler device and never taken orally. Always use one capsule at a time.
The Spiriva Respimat Inhalation Spray comes with one aluminum cartridge and one specialized Respimat inhaler. The cartridge is specifically designed for the Respimat device and should not be interchanged with other products. The inhaler features a sleek cylindrical design, a gray body, and a transparent base that can be removed for cartridge insertion, along with a convenient dose indicator.
Before starting Spiriva, discuss with your healthcare provider if you have conditions such as narrow-angle glaucoma, an enlarged prostate, or bladder obstruction.
Keep in mind that Spiriva serves as a preventative measure against bronchospasm and is not intended for treating acute attacks. For immediate relief during an attack, a fast-acting inhalation medication is recommended.
Precautions Before Use
Avoid using Spiriva if you are allergic to tiotropium or ipratropium (found in Atrovent, Combivent, DuoNeb).
To ensure Spiriva is suitable for you, inform your doctor of any medical history related to:
- Narrow-angle glaucoma
- Kidney disease
- Milk allergies
- Enlarged prostate or urination difficulties
If you are pregnant, consult your doctor, as the effects of tiotropium on an unborn baby are not well-established. However, poorly controlled asthma during pregnancy can pose risks such as premature birth or low birth weight, making the treatment benefits potentially outweigh the risks.
Discuss with your healthcare provider whether breastfeeding is safe while using this medication. Note that Spiriva Respimat is not suitable for children under 6 years old.
How to Use Spiriva
Always use Spiriva exactly as prescribed by your healthcare professional. The standard dosage is 2 inhalations via the mouth once daily. Adhere to all instructions on your prescription label, and do not exceed the recommended dose or duration.
Remember, Spiriva is not a rescue inhaler and will not provide quick relief during a COPD exacerbation. Consult your doctor if you feel your medications are not effectively managing your condition.

For children using Spiriva Respimat, ensure they are supervised by an adult during administration.
The Spiriva HandiHaler comes with capsules and an inhaler device. To use it, place one capsule into the device, close the mouthpiece, and press the button to pierce the capsule, releasing the medication into the inhalation chamber. Two inhalations are required to complete the dosage.
Do not ingest Spiriva capsules orally; they are specially designed for use with the HandiHaler. Dispose of any unused capsule immediately after removing it from the blister pack to prevent exposure to air, which can degrade the capsule.
Spiriva Respimat is supplied in a cartridge compatible with the inhaler included with your medication. Turn the base of the Respimat until it clicks to deliver the medication into the chamber, requiring two puffs for a full dose.
Be aware that the Respimat inhaler will lock after 30 doses (or 60 puffs) and features an indicator that alerts you when only 14 puffs (about 7 days) remain.
Regular testing of lung function is essential to confirm the effectiveness of this medication for your needs.
To maximize treatment benefits, use Spiriva consistently and refill your prescription before running low.
Store both capsules and the Respimat inhaler at room temperature, away from moisture and heat. Do not freeze them, and avoid keeping capsules in the HandiHaler device.
