Methimazole is an effective medication for controlling hyperthyroidism, a condition marked by the overproduction of thyroid hormones by the thyroid gland. This drug is often used prior to thyroid surgery to lower hormone levels and minimize the impact of surgical manipulation. In addition, methimazole is a common treatment for feline hyperthyroidism in veterinary medicine.

How It Works:

Methimazole functions by inhibiting the enzyme thyroperoxidase, which is essential for the synthesis of thyroid hormones. This enzyme converts iodide ions (I-) into iodine (I0), a critical step for incorporating iodine into the hormone precursor thyroglobulin, leading to the production of triiodothyronine (T3) and thyroxine (T4). Importantly, methimazole does not affect the sodium-dependent iodide transporter on the basolateral membranes of follicular cells, an action that would require competitive inhibitors like perchlorate and thiocyanate. The drug targets the site of CXCL10.

Potential Side Effects:

While using methimazole, vigilance is essential for symptoms such as fever or sore throat, which may signal agranulocytosis, a rare but serious reduction in white blood cell count (neutropenia). Should such symptoms arise, a complete blood count (CBC) with differential is required to confirm the condition, after which the medication should be discontinued. The use of recombinant human granulocyte colony-stimulating factor (rhG-CSF) may support recovery.

Other possible adverse effects include:

  • Skin rash

  • Unusual hair loss
  • Nausea
  • Vomiting
  • Altered sense of taste
  • Abnormal sensations (tingling, prickling, burning, tightness, and pulling)
  • Swelling
  • Joint and muscle pain
  • Drowsiness
  • Dizziness
  • Decreased white blood cell count
  • Reduced platelet count
  • Aplasia cutis congenita (with prenatal exposure)

Individuals who may face adverse effects include those who:

  • Use anticoagulants like warfarin (Coumadin), diabetes medications, digoxin (Lanoxin), theophylline (Theobid, Theo-Dur), and certain vitamins.
  • Have a history of blood disorders such as leukopenia (low white blood cells), thrombocytopenia (low platelets), aplastic anemia, or liver conditions like hepatitis or jaundice.
  • Are pregnant, planning to conceive, or breastfeeding. In these cases, propylthiouracil is often recommended as an alternative anti-thyroid medication. If pregnancy occurs while on methimazole, switching to propylthiouracil may be advisable. Initial studies indicated potential fetal risks associated with methimazole; however, recent research suggests this may not be definitive.
